This is your opportunity as a new employee to enroll in health benefit plans for yourself and your family. You are eligible for benefits the first of the month following your applicable wait period of consecutive employment and you must be a full-time employee, averaging at least 25 hours per week. If you do not enroll within your applicable wait period from your hire date you will have to wait until the next annual enrollment. The elections you make now will be in effect until the end of the plan year (plan year is March through February) unless you experience a government qualifying major life event (MLE) during the plan year. You cannot cancel your insurance under section 125 of the IRS tax code, unless you experience a life event and we are notified within 31 days. Qualifying major life events include: • Full-time to Part-time (24 hours a week or less) • Part-time to Full-time (25 hours a week or more) • Marriage • Divorce • Birth/Adoption of child • Gain of Coverage Employee • Gain of Dependent Eligibility/Coverage • Gain of Spouse Employment/Coverage • Loss of Dependent Eligibility/Coverage • Loss of Spouse Employment/Coverage • Loss of spouse/child • Court Order • Benefits Status Change Form • Website entry of Major Life Event: https://services.shps.net/login/regislogin.asp. Upon logging onto the website, click on the Qualified Status Change box on the left hand side of the webpage. Follow directions as listed. • Calling Regis Benefits Dept. at 1-800-492-2925, 8:00am to 5:00pm CST. • Promotion – If promoted to manager or higher, you will maintain your current Level of coverage, but will have 31 days to elect new offered Level. If no change is made you will have another opportunity at the next annual enrollment period, if eligible. It is important to enroll before your applicable wait period is up in order to avoid waiving all coverage. If you do not apply for benefits on the website or IVR (telephone voice response system) the default coverage will automatically be put into place.
